We don't have a lot of forms in the EAP.

If you're scheduling an appointment to come in and see one of our counselors, you can download our Intake Packet before you come in and save yourself a few minutes in the waiting room.

The only other form that employees occasionally use is our Release of Information Form. Everything in EAP is confidential, so we usually don't communicate with anyone. There are situations where employees do want us to communicate with someone else, and this is the form you will be signing that would allow us to communicate with someone.
